To obtain copies of your medical records or other medical information, please visit our Medical Records desk located in Medical building three, first floor lobby. If you have questions about medical fees or would like to know more about a procedure, please contact us.

•Please bring your Taiwan National Health Insurance (NHI) card,Alien Resident Certificate(ARC) card and Passport for registration.

Obtaining Medical Records

Document Request

If you require documents of your medical records, our Medical Service Information desk can provide you with copies. We offer photocopies of the following: outpatient emergency medical records, discharge summary, x-ray reports, laboratory test reports, pathology reports, and your entire medical record.

Single document photocopies will typically take 60-90 minutes to process. Requesting a medical record summary or your entire medical record may take longer to process - we will be happy to schedule a pick-up at another time or arrange for the documents to be mailed to you.

WHAT TO PREPARE

Documents you will require:

Two IDs with photographs (Passport, National Health Insurance (NHI) card, or driver’s license)
Your medical bill receipt or discharge document.
